How should we state our prayer when offering the Self-Cultivation Record Form of the Eighty-Eight Buddhas Great Repentance (LIFO)?

Dear Buddhist friend,
You can pray for repentance and the elimination of karmic obstacles relating to a specific transgression committed in this lifetime.
Alternatively, you can put forth a general prayer as follows:
“To the Greatly Merciful and Greatly Compassionate Guan Yin Bodhisattva, I, <your full name>, have accumulated <number> times of self-cultivation LIFO. May Bodhisattva and the Dharma Protectors bear witness today as I offer the merits from the recitation of LIFO to repay some of my karmic creditors and eliminate my karmic obstacles”.
